Playthrough 1 · Story on Story, and pick up everything you see
Set action difficulty to Story and puzzle difficulty to Story. This run is not about challenge, it is about collectibles and learning the map.
Pick up every new-game item, the omamori, the documents and the items that expand your inventory. Many of the 36 missables are here and they do not come back.
You finish with Ending 1, which pops on its own. And you take Thankful to Be Here and Intellectual Pursuits along the way.
From the very first playthrough: do not use red capsules. Three of the five endings require never having touched one, and it is the mistake most people make without noticing.
Playthrough 2 · New Game+ with puzzles on Hard
Raise puzzles to Hard and start New Game+. Here you unlock endings 2, 3 and 5, and pick up the new collectibles that only appear on NG+.
Ending 5 is the UFO one and it is done separately, by listening to the radio broadcast and reading the movie posters. Careful: that ending does not validate the difficulty trophies, so do it on a partial run and do not count it as a serious playthrough.
Playthrough 3 · Everything maxed out and Ending 4
Final run with action and puzzles on Lost in the Fog, both at maximum. That way you take Clear Skies and Puzzle Master in the same playthrough instead of across two.
And this is where Ending 4 goes, the only gold trophy in the game: you need two full endings beforehand, no red capsules, purify the Sacred Sword and offer the Brooch to the Jizo Statue.
It is the playthrough where most people stop: only 22.39% clear the top difficulty.

With the three steps done you should either have the platinum or be two or three loose feats away. Go through them in the trophy menu and decide whether a fourth partial run is worth it or whether loading an earlier save is faster.

14 trophiesThese 14 trophies are not earned: they happen. They are automatic story unlocks, with no condition, no action and no way to miss them. If you are playing, they drop on their own.
That is why their names are cryptic lines that never say where they pop: Konami wrote them that way so as not to spoil the plot, and no guide publishes their location because there is nothing to do with that information.
What is useful is the order, and we derived that ourselves from the data. Story trophies always unlock in the same sequence, so the percentage of players who have each one drops strictly. They are listed from most common to least, which is exactly the order they will pop for you.
Use it to place yourself: if you have Shell of the Past but not The Farewell Party, you know you are right in that stretch. And the big gaps between one and the next tell you where people quit.

4 trophiesThis is where the real game starts. The first ending pops on its own, but the four that give a trophy have to be triggered, and their conditions lock each other out: they revolve around whether you use red capsules, what you do with the Sacred Sword and whether you offer the Brooch.
Look at the collapse: from the 65.13% who finish the story you drop to 29% on the first extra ending. That is where people decide whether they are going to replay Silent Hill f three times or leave it.
