The 23 health items in Ninja Gaiden 4: 11 in chests, 12 as rewards

Ninja Fortitude is not bugged: half the items are not in chests but come from completing missions, and four only exist in New Game+. Here are all twenty-three.

Why it looks bugged and is not

It is the game's most repeated complaint: "I combed every chapter, I have every chest and the trophy will not pop." It is not broken. The catch is that Yakumo's health does not work like a chest collectible.

Yakumo does not find ready-made health upgrades. He finds two separate materials — a scale and a herb — and the game combines them by itself the moment you have one of each, extending your bar. You will never find the assembled item. That is why it takes 20 items for Yakumo's 10 upgrades.

Ryu works the old way: his item is a single one, and there are three, in chests within his three chapters.

20 + 3 = 23 items, giving 13 health upgrades. And of those 23:

- 11 are in chests. These are the ones any collectible guide covers. - 12 come from completing missions. They are not hidden anywhere: they drop on hand-in.

That is where almost everyone goes wrong: combing the map for twelve chests that do not exist.

On top of that, four of those twelve missions only appear in New Game+ — the four flower kunai ones. The trophy cannot be closed in a single run, whatever you do. 17.71% have it.

Key

Accept every mission each time you use a terminal, and hand them in at the next terminal you see: several are continuations of others and will not appear until the previous one is turned in.

The 11 in chests

Eight are Yakumo's and three are Ryu's. All of them sit on detours off the main path.

2YakumoAfter the big fight against the flying enemies and the surveillance drones you reach a new monorail track. Instead of riding it, turn left: the chest is right in front of you.
3YakumoAfter your first lantern daemon encounter, turn right and go up the narrow stairs. The chest is right at the top.
4YakumoThere is a stealth area with three officers right after you smash a wooden barricade. When it is done, go left instead of following the story to the right. Wall-run up: the chest is in front of a huge tengu statue.
6YakumoRight at the start of the chapter you land on a rope bridge. Instead of crossing it, head the other way: the chest is on the broken bridge, to the left of the shops.
9YakumoIn the large surf area you have to pass under a big orange gate to reach the next one. As soon as you slide under, hug the left side and keep surfing to a tunnel. It ends in a small cave where enemies spawn: kill them and open the right-hand chest of the two.
11YakumoYou reach a station where you climb to a second level and ride a single track again. You will glide past a station on your left that you pull yourself onto with the wire. Four chests in a row there: it is the last one.
12YakumoIn the room where you kill the chapter's first golem samurai. In the right-hand corner, before going through the next door.
19YakumoAfter beating Ryu you start climbing. When you reach the first shops you hit a shaft you would bird-flip up: instead, walk around the shaft's left corner and the chest is there.
15RyuAfter fighting your way through the disco sewer area you land in a circular room with a shop in front of you. Turn a full 180°: the chest is against the wall.
16RyuOnce you have used the four drones to cross the big gap you end up right under the doll head. Instead of going straight, take the left path and swing across another gap. In that small area where you kill the kettle fiend, look left: the skill chest is up a few steps, and Ryu's item is in the far corner from there.
18RyuIn the room with the triangular steel beams, cross the bridge in the middle and turn right instead of left. The chest is in front of you.

The 12 the missions give you

These twelve are in no chest at all: the game hands them over when you turn the mission in. Eight come on the first run; the four flower kunai ones only exist in New Game+.

Eight of the twelve are also "find the item" missions, so the mission itself is already a hunt: the health reward comes afterwards.

If you finished the first run with every chest and every chapter mission, you are missing exactly four items: the four flower kunai. That is two bar extensions, and it is precisely what separates the people who have the trophy from the people who think it is broken.
