How to stack the Resident Evil Requiem challenges and not play it four times
Insanity difficulty, no healing, no blood collector and under four hours. Three of them go together; one does not. This is the split that works.
And it counts towards Never Touch the Stuff, Minimalist, Speed Demon, Bloodlust, required for the platinum.
Two runs, not four
Resident Evil Requiem has four run-wide conditions that at first glance look like they demand four playthroughs. They do not: there is only one real conflict.
Bloodlust asks you to collect 5000 microsamples of blood with the collector. Minimalist asks you to finish the story without Grace using it once. Those two do not fit in the same run.
Everything else stacks.
The difficulty trophies stack. A single Insanity run also gives you Rookie Agent, Rising Agent and Resolute Agent.
The split we recommend
This is how we would lay it out at EveryTrophy:

The second run bundles four hard trophies at once, and that is why its plan is so strict: no herbs, no blood collector and under four hours on the clock.
Why the first run is best done on Classic
There is a specific reason: The Ol' Fashioned Way asks you to save at a typewriter with an ink ribbon, and ribbons only exist in Classic mode. On Modern the game saves for you and that trophy never shows up.
That explains why only 60.97% have such a trivial trophy: plenty of people play on Modern and never get to see it.
On top of that, Classic already gives you Resolute Agent, so if something goes wrong on the Insanity run you are not left with nothing.
And if you would rather play it safe
Cramming four conditions into the same Insanity run is demanding. A sensible alternative: do the Insanity run without the four-hour limit, at your own pace, and leave Speed Demon for a short third run on Casual.
The four-hour trophy does not require any particular difficulty, and on Casual with the route known you have time to spare. 44.50% have it, more than Insanity itself.
