The codes are fixed, but they change with difficulty

There are 5 safes in Resident Evil Requiem, and all of them are in Grace's sections. None fall in Leon's.

Two things worth knowing before you open the first one:

You do not need the file. The game wants you to find a document with each safe's combination, but the codes are always the same and work even if you have read nothing. You can open all of them the first time you see them.

And here is the detail that ruins second runs: the codes are different on Insanity. It is not that the file has moved: the combination itself changes. Go in with the normal-run numbers and the dial will spin back to zero over and over, and you will assume you misunderstood the mechanic. Which is why we give both columns.

How they open: each combination is three numbers and a turn direction. You spin the dial left or right to each number, in the given order, then confirm. Get a number or a direction wrong and the dial resets itself to the default position, and you can try again with no penalty.

Key

In Requiem everything is missable: there is no chapter select. If you want to be able to come back for something, keep a manual save as you enter each area. It is the only safety net you have.

Safe 3 deserves priority even if you are not after the trophy: inside is the document that marks every Mr. Raccoon on your map. Opening it early turns that other collectible into a stroll.

The 3 BSAA containers, all Leon's

The other container trophy is the exact mirror of the safes one: the 3 BSAA containers are only in Leon's sections, in the East Raccoon City area. They are big blue shipping containers, and they mark themselves on the map as you get close.

First you need the BSAA Container Keys, and they cannot be missed: they are handed to you in a mandatory story cutscene. The sequence is this: push on to the Underground Parking Garage, where a flooded gate has a panel asking for two batteries. Both are story items — one in the garage's north-west corner and one in the Sewage Facility — so you will pick them up anyway. Inserting them opens the gate and you come out on a street: go left towards the entrance to Cedarbrook Apartments and trigger the cutscene that gives you the keys.

With the keys in hand, all three can be opened immediately:

Container 1 — At the east end of the main road, between the Gas Station and Cedarbrook Apartments. At the end of the road, climb up behind the truck.

Container 2 — In Central Camp, in the corner next to the tent. This one is mandatory for the story: inside is the Repair Kit you need to fix the Broken Signal Receiver. Even if you are not chasing the trophy, you will open it.

Container 3 — The most hidden. On the main road between the Gas Station and Central Camp, use the fuel can on the generator at the big crane. A gate opens: cross the construction site to a ladder leading underground, and follow the path until you come out at the start of Raccoon City. The container is outside The Applegate Hotel. You walked past it earlier, but without the keys you could not open it.

Key

If you are missing one, it is almost certainly 3: it is the only one that needs a long detour through the construction site and the underground, and since it sits in an area you already walked through, it is easy to write that area off as seen.
