All the risk sits in one trophy. Like a warrior asks you to unlock the six mastery branches, and that takes 165 of the run’s 200 resonance points. Resonance points are the only collectible that does not carry over through chapter select: finish on 150 and the only way out is a fresh run.
Everything else is recoverable. Relics, blades, charms and echoes can be picked up from chapter select, and you can leave the chapter the moment you have them.

Which trophy is missable, and why can’t you get it back?
Like a warrior, which asks you to unlock the six mastery branches. Buying them all takes 165 resonance points, and the run contains 200: the margin is 35.
The reason it does not come back is specific: resonance points are the only collectible that does not carry over when you replay a chapter. Relics, blades, charms and echoes do. The points do not.
So if you finish the campaign below 165 there is no shortcut: you start a fresh run. There is no New Game+ either.
Which is the hardest trophy?
It depends on what you mean by hard, and both answers are useful.
By percentage, the collectibles: Hoarder and Superstitious sit at 2.0%. But there is no difficulty there, there are 67 objects to find.
By execution, Untouchable: six perfect parries in a row, and the description does not say it, but they have to fit inside two or three seconds. You farm it at the start of Chapter 10 with difficulty set to Nightmare — so the enemies are aggressive — and invincible mode on, which blocks nothing.
And the one most people fail without knowing why is Excessive force (2.7%): you have to let a Corrupted grab you *with the focus meter already full*, and the instinct is to spend it beforehand.
