Mysteria Ecclesiae: eight trophies, all missable, and one asks you to die in the most absurd way
Ostraconophobia asks you to let a crayfish kill you and sits at 3.09%. Eight trophies from the third DLC, none counts towards the platinum, and all eight are lost if you finish the content without them.
And it counts towards Holy Relic, Better Safe Than Sorry, Feline Affection, Survivor, Stop Licking That!, Ostraconophobia, Quiet as a Church Mouse, required for the platinum.
All eight are missable, and that changes how you play
Mysteria Ecclesiae is the third story DLC, and it has a quirk the other two do not share: all eight trophies are missable.
That means there is no net. Once the DLC ends, whatever you did not take is out of that run, and none counts towards the platinum, so the question is not whether you can recover them: it is whether replaying is worth it.
Fresh Air — 8.83%. Complete the DLC. Silver, missable — even the one for finishing it — and the block's ceiling.
The practical consequence is simple and applies to all eight: this DLC is played with the trophy list open. It is not content to discover as you go if you want all eight, because half of them are gestures nobody would think of unprompted: talking to the same character repeatedly, petting a cat, tasting flour you should not.
And three that consist of doing strange things on purpose:
Feline Affection — 4.52%. Interact with a monastery cat several times. It sounds like a joke and it is, but only 4.52% have it: you have to remember.
That's Not Food! — 8.66%. Taste the flour that had a corpse's hair in it. Black humour, hidden and missable.
Survivor — 5.58%. Listen to all of Brother Vojtěch's jokes. You have to go back and talk to him several times across the DLC, not once: it is a trophy lost by not persisting.

Ostraconophobia: the most absurd trophy on the list
Ostraconophobia — 3.09%. Die at the claws of a crayfish.
At 3.09% it is among the game's rarest, and it is unquestionably the most absurd trophy on the entire list. There is no technique and no secret location: you have to let the crayfish kill you on purpose, at very low health.
That implies two things worth planning. First, lowering your health deliberately, because a healthy Henry does not die to pincers. Second, doing it when dying does not matter: it is a real death, with the reload that comes with it.
The sensible move is to take it just before a save, or at a moment when the run has nothing to lose.
And the trophy at the opposite extreme of the same DLC:
Silent as a Snake — 6.86%. Finish the DLC without the knights having to carry you to the infirmary even once — that is, without being defeated. Silver and missable.
The two together are a good picture of this content: one asks you to die in the most ridiculous way possible and the other asks you never to fall. They are not incompatible — death by crayfish is not a combat defeat before the knights — but it is worth being clear on them so as not to confuse the two.
And the remaining two:
Sacred Relic — 9.06%. Find a sacred relic. Silver, missable and hidden.
Better Safe Than Sorry — 8.62%. Craft yourself a plague mask. Missable: it has to be made at the point where the DLC allows it, not whenever you remember.
