Purifying the sword opens Ending 4 and closes Ending 3: they are separate runs

Two puzzles that allow no trial and error, the item chain that decides three of the five endings, and the eleven areas to sweep for the rarest exploration trophy.

Two one-shot puzzles

Acupuncture Specialist — 39.81%. Reach the Nishida residence without a single mistake in the scarecrow puzzle. The condition starts when you reach the Fields after speaking with Shu.

It is solved by choosing the right dialogue options, and one mistake triggers combat: at that point the trophy is gone. Look it up before going in, because it allows no trial and error.

Sly Like the Fox — 33.41%. Obtain every emblem without a single mistake in the Inari statue puzzle. The condition starts when you enter the shrine room with several doors.

Same philosophy as the scarecrows: it is a one-shot puzzle. There is no way to probe it.

These two are the only trophies in Silent Hill f that punish improvising, which is why they are the ones worth having written out before you reach their area.

Winners Don't Do Drugs — 32.81%. Reach the Iwai residence without having taken a single pill, and also leave the pills on the table in Shu's workshop.

There are two conditions, and the first is cumulative from far earlier: it is easy to get there and discover you already broke it without noticing. It goes hand in hand with the red capsule rule: if you leave them alone for the endings, this one comes free.

Careful

The scarecrow and Inari statue puzzles allow no trial and error: one mistake loses the trophy. Look them up before entering their area.

The sword chain and the trophy that demands sweeping eleven areas

Four feats form the chain that decides three of the five endings, and the order between them is not negotiable.

The Malevolent Blade — 32.00%. Obtain the sacred sword after discovering its legend. Order matters: the legend first, then the sword.

It is the chain's first link, so it is not optional however much it looks it: without it there is no Ending 3 and no Ending 4.

Treasured Belonging — 31.36%. Obtain the small crystal vial with the Agura no Hotei-sama. It is the item that unlocks Ending 2, so it is tied to that specific run.

Be Free of Your Hatred — 29.93%. Purify the sacred sword.

Watch the ending order: purifying it is required for Ending 4, but it closes off Ending 3, which demands it unpurified. They are separate runs and there is no way to do both in one.

A Gift I Cannot Accept — 26.22%. Offer the brooch to the old Jizo statue.

It is Ending 4's final condition and therefore the game's only gold trophy's. It is among the least common feats precisely because only those on their third run get there.

That fall from 32.00% to 26.22% is the whole chain in numbers: each link sheds a handful of people, and only one in four holds the last.

Close to the Sun — 24.14%. Max out health, sanity and stamina.

They are raised with the ema you find across the areas and redeemed at the hokora. There is no shortcut: you have to sweep all eleven areas collecting them.

It is among the list's rarest, and not through difficulty but because it demands complete exploration. It pays to start collecting ema from the first run even without redeeming them: upgrade progress is the one thing in this block that does not depend on which ending you are chasing.
